X-Ray Findings. Early singns are soft tissue swelling, osteoporosis around the involved joint, and erosion of the peripheral "bare space" of bone surface that is not covered by cartilage. Later, extensive erosion of cartilage causes joint space narrowing. Bony cystes result from invasion by granulation tissue. Rheumatoid synovial may invade the joint capsule, ligaments, and tendons, and it may add to joint instability include by destruction of cartilage and bone. After some years, the degenerative changes of secondary osteoarthritis may be superimposed.
Special attention should be directed to the upper cervical spine and especially to C1-2, where subluxation may result in the development of serious neurologic complications.
